Change Proposal Evaluation Plan to Congestive Heart Failure Patients

The successful implementation of a change proposal requires a number of initial steps that will cement the likelihood of success of the proposal. It is vital to collect relevant and accurate data concerning the issue, which is exacerbation in patients with CHF. It is important to communicate the importance of the required change and to present it in a way that is understandable and accessible (Strata, 2018). The proposal will focus on the information being accessible to CHF patients, doctors, and nursing staff in order to avoid exacerbation. The resources can be available through online sources, information leaflets, and direct communication in order to involve patients in inquiries.

The objectives of the proposal would be to increase the available resources concerning the issue of exacerbation in CHF patients and as such, to improve their knowledge of how to avoid dangerous exacerbation. The resources should be peer-reviewed, tested, and easily accessible to patients. As such, it is expected that some outcomes of the proposal will include decreased cases of exacerbation of CHF patients in the area and better response rates and methods in case of an exacerbation.

The evaluation process will be primarily relying on the collected data after the implementation of the change proposal as well as the response of patients with CHF. Firstly, it is essential to outline criteria that will be used to assess the results of the change proposal (Fiorio et al., 2018). The criteria for the following proposal will measure success by cases of exacerbation in CHF patients prior to and after the proposal. Additionally, positive responses in patient interviews will indicate whether the provided information was clear, accessible, and otherwise useful. In order to accurately estimate which format of the information was the most effective, it was suggested that the interviewed patients themselves communicate which format they encountered and preferred. Additional interviews with doctors and nursing staff of the patients can help determine the relationship between the proposal and cases of exacerbation. The evaluation process would also take into consideration the feedback provided by the patients for further changes.
