The proposed date and location of the assessment
The proposed date for this assessment will be December 1, 2018. The respondent will be encouraged to sign an informed consent form (Mackey & Bassendowski, 2017). The best location is the interviewee’s home. I will visit the individual at their place in order to maintain the highest level of privacy. The information obtained during the interview will be unavailable to other people. Additionally, the respondent’s real name will not be indicated.
The process of documenting the assessment and steps to ensure its accuracy
Interviewers can use different methods to document the information obtained from the identified respondents. During this assessment, I will consider these key steps: compose questions, get the interviewee’s responses, take notes, and ask for verification. The first stage will ensure that the designed questionnaire captures every issue. The next phase is to visit the individual’s home in order to complete the assessment process (Harkess & Kaddoura, 2016). The information will then be summarized using relevant entries. The final (follow-up) stage is to ask the respondent to verify the presented information.
