Mental Disorders: Preliminary Care Coordination Plan

Health is not merely the absence of disease and bodily defects. It is a state of complete physical, mental and social condition. Therefore, it is apparent that three health segments: body, soul, and society, are closely interconnected and affect well-being. Individuals who are balanced and resistant to stress are wholesome and more prosperous in life. They are active, have enough energy to work, take care of their loved ones, set a positive example for others, and create a favorable climate in the family. The ability to resist stress, fight anxiety, reduce the likelihood of psychosomatic diseases. Still, unfortunately, the contemporary rhythm of life is such that mental health is exposed to damaging influences.

There is a considerable amount of stress at work and home simultaneously. The number of individuals with mental disorders in the world is increasing, and because of this, by 2030, the global economy will lose 16 trillion dollars (Bradvik, 2018). Some of this amount is the direct cost of medication or other treatments, but the significant losses are indirect (lost productivity, social security, and law enforcement). Scientists estimate that the number of mental illnesses in the world has increased dramatically over the past 25 years, and there is no country investing enough to deal with the crisis (Bradvik, 2018). The reasons for this dynamic are an aging society, massive social disruption caused by economic concerns, civil conflicts, wars, and violence in places such as the Middle East, Central America, and Africa.

Furthermore, epidemics such as the Ebola outbreak in West Africa and natural disasters correspondingly affect the psyche. The topic of mental health and discrimination concerning mental health crises is a growing concern. The WHO estimates that at least 10 percent of the world’s population has mental disorders, including 20 percent of children and adolescents (Bradvik, 2018). There is also a direct link between mental disorders and the most costly chronic diseases to treat, including cancer, cardiovascular disease, diabetes, HIV, and obesity. Meanwhile, mental disorders account for 30 percent of all nonfatal conditions and 10 percent of the total (Bradvik, 2018). Thus, mental health is a significant matter for the well-being of populations, regardless of age, gender, race, socioeconomic and political status.

In recent years, numerous procedures and effective practices have emerged in psychotherapy. Most studies indicate that the treatment method using both medication and psychotherapy for major psychiatric disorders is effective (Brouwers, 2020). By creating an atmosphere of empathy and encouragement, the therapist can help the person identify the source of worries and consider possible solutions to them. Moreover, emotional awareness and understanding that the patient receives through psychotherapy, leads to a change in attitude and behavior, allowing the person to live a fuller and more satisfying life. Problem-oriented therapy is also an appropriate and effective treatment method for every patient who is experiencing hardships.

Care coordinators are significant actors who can enhance both short-term and long-term indicators of the mental health improvement continuum strategy. A mental wellness care coordination plan strives to improve the patient’s condition with all the community’s resources. Its primary emphasis is on the core competencies, grounds, and basics of organizing all varieties of means, objectives, and interdisciplinary interventions to promote rapid and quality developments. Such a plan mandates an awareness of the diverse characteristics of mental health, its heart symptoms, and the steps to improve the continuum of care. Multiple factors impact cognitive health, including upbringing, prescriptions, genetics, and biology. The competencies identified in mental health most promising approaches require a coordinated approach, especially when employing patient-centered care strategies. Correspondingly, coordinating household members and developing mutually agreed-upon objectives for medical interventions is an integral part of thorough and integrated care.

A pre-plan of care coordination for mental illness includes utilizing public resources founded on patient well-being and developments, including estimating desired results. Providers should promote the most profitable practices constant with patient-centered models, including enveloping oneself with optimistic thinking. Proper self-image, assessment, understanding available resources, and knowing one’s liberties are equally valuable. These most reasonable practices will influence a positive coordinating plan and impact patients appropriately.

Interventions and strategic measures correlated to mental health are vital and obligatory aspects of the coordination plan. They are paramount to enhancing people’s well-being and guaranteeing that patients access the essential information and resources. The focus of the care plan is on delivering therapy for patients with cognitive disorders and improving their knowledge of their illness and self-care initiatives (Brouwers, 2020). Mental health promotion includes actions that promote psychological health, and creating a favorable environment is among them.

There should be a world where individuals protect all fundamental civil, political, socioeconomic, and cultural rights, as it is the foundation of mental health. It is challenging to preserve well-being without safety, independence, and other vital aspects of these privileges. Therefore, national mental health policy should concentrate on mental disorders and the more comprehensive elements that elevate mental health. Moreover, it is critical to identify permanent risk factors such as misinformation, inaccurate cultural and social practices.

Another goal is to involve family and other community members in addressing health risk factors and encouraging patients to adopt models of care that fit their understanding. A preliminary care coordination plan seeks to determine the most continuous risk factors, including those in the home, work, or community environment. It also recognizes potential community resources to address the concern within the desired framework. Nurses and other providers should determine measures that can enhance the management of mental health difficulties (Graham et al., 2019). They should implement a holistic health care practice that incorporates family and community resources in an integrated framework. Only the combined efforts of the patient and family will be the proper response to health challenges.

A secure and sufficient continuum of care demands to acquaint and inform patients about mental health resources in their communities. These include governmental and nongovernmental programs to improve their well-being. Moreover, patients should be able to communicate with mental health providers who can introduce programs and resources in their states (Moreno et al., 2020). For example, it is reasonable to familiarize them with non-profit organizations that deliver mental health cures and therapeutic services. Through such associations, community outreach endeavors expand access to resources, coordinate movements, and construct a network that fosters mental health outcomes.

Furthermore, community services and support groups are critical resources that introduce patients to advanced aids and services, helping them with knowledge and data. Patients can also resemble their conditions’ developments with others, including the closest available treatments and access virtual and digital databases such as National Mental Health Groups and Associations National Coalition for Mental Health Recovery (Brouwers, 2020). These resources serve as a reliable source of information and promote understanding of the significance of comprehensive treatment and its implementation.

Thus, the primary goal of the advanced care plan is to improve the condition of patients with mental disorders. It aims to create a favorable atmosphere, involving the patient’s family in the treatment process and a comprehensive study of the grounds of the concern. Correspondingly, the plan includes all available resources of the society, among which the primary ones are organizations and charitable communities that are engaged in the treatment and prevention of mental health problems. All these elements will be coordinated through a patient-centered approach, guaranteeing quick and qualitative effects.
